It should not cause problems unless there were other complications I don't know how sick she got, but if you guys are cousins (blood realted) and wanting to have children together please consider talking to a doctor about having a genitic workup babies can be born with some significant complications in these types of marriage. (Suppositions:) Unless the surgeons who performed the appendectomy accidentally messed around the ovary. But the more plausible explanation would be that she is already infertile to begin with but only a proper medical practitioner (obstetrician) would be in a position to say that.
I hope your cousin is adopted! if she's not, I would strongly recommend that you do not concieve children and adopt anyway, because pro-creating with your first cousin can cause a very very great number of birth-defects.
